This form has been developed to allow learners who were enrolled at Achievement Training Limited to provide a self-declaration of their employment outcome.

Please note: This form only requires completing if you were unemployed when starting your programme at Achievement Training (as a result this form should not be completed by learners who were enrolled on an Apprenticeship programme).

By signing this statement I confirm that the details held within this self-declaration are accurate at the time of completion.
