Photography Permission Form

As part of our communications activity, Achievement Training occasionally uses photography for publicity purposes. We would like your permission to photograph you/your relative for possible inclusion in our publications, website and other publicity material. The image(s) will remain the property of Achievement Training and will be used for the designated purpose of promoting Achievement Training's aims in relation to access to further education. It may also be included in the central Achievement Training image library for use by other Achievement Training practitioners.

By signing and submitting this form I permit Achievement Training to use photographs of me/my relative in Achievement Training publications and publicity material and for inclusion in the central Achievement Training image library. Where applicable (under the age of 19) I have received permission from the parent/carer detailed above.
